What we know about Sanae is that she's the current ōhōri of the Suwa clan (per an interview with ZUN), their living god and representative of Suwa-Daimyōjin who in Touhou is primarily Kanako. She's the custodian of Moriya Shrine and has been it's owner since she was a child. Since Kanako is actually real and Sanae is deeply acquainted with her already (unlike with Suwako) you can assume that she's been talking to and interacting with Kanako as long as she can remember.
Sanae and Moriya Shrine as a whole is a synthesis between the Suwa clan, the Moriya clan, and the Suwa Taisha and Moreya shrines, so Sanae is both the living god and ritual keeper of the Shrine.
Parents usually stepped down when they were old so they could mentor their children, the living god especially needed to be pure and childlike because they were to be the representative of the clan's chief god, so Sanae's parents were likely very much still alive when she was young so they could pass the rituals down to her. Given that she doesn't mention them at all or think about them in Gensokyo, I'd wager that they were so old that they're now dead, and Sanae was raised primarily by her god. It makes sense, after all, that someone who has unwavering belief in a deity would be brought up by them and become like a second parent to them.
She probably had friends, but she's a nerd and even sympathizes with Sumireko once over a shared annoyance with expectations at school (Grimoire of Usami), so she must've been an outcast, she's a nerd and a religious nutjob who probably wore the frog and snake hair ornaments to school, too, and was proud of being a god. People would think she's a weirdo. She leaves the outside world with only a little bit of uncertainty and chooses to do so willingly. She could've stayed, but she didn't. I don't think she had very many things tying her down.
I see her coming to Gensokyo as her finally being allowed to be her nerdy self and everyone accepting her for it. It lives up with ZUN's more positive outlook on Gensokyo that he had back then.
